Peripheral Nerve Surgery

Specialist assessment of peripheral nerve injury and compression.

Peripheral nerves carry movement and sensation between the central nervous system and the limbs or face. Injury may cause pain, numbness, weakness or loss of function—and sometimes requires an approach spanning neurology, orthopaedics, hand surgery and rehabilitation.

When should a peripheral nerve problem be considered?

Symptoms vary according to the nerve and level involved. Mapping their distribution helps distinguish a local peripheral nerve lesion from a cervical or lumbar root, plexus or central nervous-system condition.

Sensory change

Numbness, tingling, reduced sensation or marked sensitivity to touch.

Neuropathic pain

Burning, electric-shock or shooting pain along a nerve territory.

Weakness

Loss of grip, foot lift, shoulder movement or a specific motor function.

Scar-related pain

Focal tenderness after injury or surgery may suggest a symptomatic neuroma.

Not every pain is nerve pain, and not every nerve injury needs surgery.

The first aim is to localize the problem, define its severity and select the appropriate route: observation, rehabilitation, pain management or surgery.

How is a nerve lesion assessed?

Assessment starts with the history: onset, preceding trauma or surgery, pain distribution, aggravating factors and functions lost. Examination maps sensation, power, muscle balance and provocative findings.

Depending on the clinical question, additional tests may include nerve-conduction studies and EMG, high-resolution nerve ultrasound, MRI or targeted diagnostic blocks. A test result must be interpreted in the context of the examination.

What treatments may be considered?

Treatment is determined by diagnosis, nerve continuity, severity, time and the patient's functional priorities. Options may include activity modification, splinting, rehabilitation, medication and observation. Surgery may involve decompression, neurolysis, direct repair, nerve grafting, nerve transfer or management of a symptomatic neuroma.

When meaningful reinnervation is no longer expected, tendon transfer, free functional muscle transfer or other reconstructive strategies may be considered in selected cases.

Why does timing matter?

Nerves regenerate slowly, and denervated muscles have a limited capacity to receive new axons. Urgent evaluation may be needed after an open nerve injury, rapidly progressive weakness or acute major trauma. Other conditions can be assessed electively, but prolonged motor denervation may narrow reconstructive options.

What should be brought to consultation?

  • Operative reports and prior clinical summaries.
  • EMG and nerve-conduction reports, including earlier studies for comparison.
  • Relevant imaging with the original files where possible.
  • A concise timeline of symptoms, treatments and functional changes.
